Mental Status Exam (MSE)
This is conducted at the time of the

biopsychosocial spiritual assessment.
The mental status exam is a recording of observations used to establish changes in the client over time. At each meeting the therapist notates the appearance, speech, thought process and content, mood and affect of the client. It includes screening for suicidal, homicidal ideation and follows with a check on sensory perceptions, these would be indications of illusions – delusions or hallucinations. The client is asked about hearing things, seeing things or feeling things that other people don’t see, hear or feel. This can include tactile, Phantosmia (olfactory) audio or visual hallucinations.
